Job Summary:The Medication Reconciliation Technician plays a critical role in ensuring patient safety by accurately collecting, reviewing, and documenting medication histories. This position supports pharmacists and other healthcare providers in identifying and resolving discrepancies in medication records during transitions of care, such as admission, transfer, and discharge.________________________________________Key Responsibilities:Obtain and document accurate medication histories from patients upon admission.Going into patients rooms with doctors and nurses to obtain history (patient facing role)Review and compare current medications with newly prescribed ones to identify discrepancies.Collaborate with pharmacists, physicians, nurses, and outpatient pharmacies to clarify medication information.Assist in updating allergy information and medication lists in electronic health records (EHR).Educate patients and caregivers (families) on medication changes, usage, and potential side effects.Ensure medication lists are accurate during transitions of care.Update accurate medications in EMR (epic)Calling pharmacies to cross check medicationsSupport inventory control and medication labeling as needed.We are a company committed to creating inclusive environments where people can bring their full, authentic selves to work every day.
