Job Description

The Registered Nurse Coordinator is responsible for providing high-quality nursing care, patient education, and care coordination for individuals living with bleeding disorders. This role serves as a key liaison between patients, providers, hospital staff, and community resources while supporting treatment planning, clinical services, and ongoing advocacy efforts.

The Nurse Coordinator conducts nursing assessments, provides telephone triage, develops and maintains treatment plans, administers medication infusions, assists with comprehensive clinic visits, and educates patients and their families on bleeding disorders and treatment options. This position also participates in an on-call rotation and works collaboratively within a multidisciplinary healthcare team to ensure exceptional patient outcomes.

Schedule
Full-Time (30 hours/week)
Four 10-hour shifts
Monday through Friday
Typical hours:
6:45 AM - 5:15 PM OR
7:00 AM - 5:30 PM
On-call rotation: 4-5 days every other month
1-2 holidays per year

Required Skills & Experience

Active Registered Nurse (RN) license in Wisconsin or reciprocal state
Graduate of an accredited nursing program (Associate Degree or BSN)
Current CPR certification for Healthcare Providers
Minimum 3 years of nursing experience
EPIC EMR experience
Experience conducting nursing assessments and patient triage
Strong care coordination and patient advocacy skills
Ability to develop, implement, and update patient treatment plans
Experience educating patients and families regarding medical conditions and treatment plans
Strong documentation and charting skills
Excellent communication and organizational abilities
Ability to work independently and as part of a multidisciplinary team

Nice to Have Skills & Experience

Bachelor of Science in Nursing (BSN)
1+ year of outpatient or clinic nursing experience
Hospital-based nursing experience
IV infusion experience
Experience working with chronic disease management programs
Previous experience with hematology, bleeding disorders, or specialty care populations
Experience coordinating care between multiple providers and healthcare organizations
