Job Description
The Care Coordinator (RN or LPN), based in Wisconsin and under the direction of the Director of Clinical Operations will manage and coordinate the health care for friars in Mt Calvary, Milwaukee, Chicago, and as needed in Montana. This position will serve as a resource to the guardians of these Capuchin communities as well as the Director of Clinical Operations. This role is a mix of clinical coordination, case management, and relationship-building—supporting holistic wellness while preserving dignity and independence.
Key Responsibilities
•Coordinate and manage healthcare plans for friars with acute and chronic conditions
•Develop, document, and monitor individualized plans of care
•Provide education and guidance on disease management and specialty care referrals
•Conduct routine health check-ins (at least every 6 months; more frequently as needed)
•Support hospital discharge planning and ongoing aftercare coordination
•Assist with care transitions across healthcare settings (home, hospital, skilled nursing, etc.)
•Maintain accurate medical records, including emergency health documentation
•Track appointments, procedures, outcomes, and Power of Attorney records
•Accompany friars to medical visits when clinically appropriate
•Communicate regularly with friars, guardians, and leadership regarding care plans and outcomes
•Build relationships with hospitals, clinics, and long-term care providers
•Submit weekly updates to the Director of Clinical Operations
•Participate in professional development related to geriatrics, wellness, and senior care

Required Skills & Experience
Active RN or LPN license
5+ years of experience as RN/ LPN
2+ years in geriatric, long-term care, or skilled nursing
Ability to travel to Milwaukee, Mt. Calvary, Appleton, and Chicago
Ability to maintain confidential information and preserve the dignity of friars
Effective Microsoft Office skills
Effective communication, problem solving, interpersonal and organizational skills
Nice to Have Skills & Experience
Experience in supervisory nursing or MDS (Minimum Data Set) roles
