Job Description

We are seeking a compassionate and experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our client's program. The role will begin on a part-time basis (first 2–3 months) while our patient base expands and will transition into a full-time position. The NP/PA will provide medical care for seniors and diabetic patients inside the mobile clinic van parked at the patient’s home address.

Key Responsibilities
- Perform comprehensive health assessments in the mobile clinic van at patient homes.
- Provide ongoing care and management for diabetic patients and seniors with multiple chronic conditions.
- Order and interpret diagnostic tests as appropriate.
- Prescribe medications and manage treatment plans within scope of practice.
- Develop customized nutrition and lifestyle plans tailored to each patient’s needs, especially for diabetes management and healthy aging.
- Educate patients and families on diabetes management, chronic disease prevention, nutrition, and healthy lifestyle choices.
- Document visits accurately and maintain up-to-date patient records.
- Collaborate with physicians and care team members to ensure continuity of care.

Required Skills & Experience

- Active Nurse Practitioner license (Texas) or Physician Assistant license (Texas).
- Master’s degree or higher from an accredited NP or PA program.
- Certification in Family NP, Adult-Gerontology NP, or PA-C (preferred: experience in geriatrics and/or diabetes care).
- Minimum 2 years of clinical experience in a primary care, geriatrics, or diabetes-focused role.
- Strong interpersonal and communication skills, with a passion for working with seniors.
- Comfortable delivering care in a mobile clinic setting.
- Valid driver’s license and reliable transportation for personal travel (note: NP/PA will not be responsible for driving the mobile clinic).

Nice to Have Skills & Experience

- Bilingual in English and Spanish a plus
