Job Description
The Registered Nurse Case Manager is a key member of the interdisciplinary hospice team, working under the general direction of the Medical Director and/or attending physician. This role is responsible for skilled nursing assessment, care planning, coordination of services, and ongoing evaluation to maximize comfort and support for patients and families throughout the end‑of‑life journey.
Day‑to‑Day Responsibilities
• Manage a patient and family caseload, including assessment, planning, implementation, and evaluation of nursing care
• Perform comprehensive initial and ongoing assessments addressing physical, emotional, psychosocial, and spiritual needs
• Collaborate with physicians, hospice team members, and outside agencies to coordinate optimal patient care and resource utilization
• Develop, communicate, and update individualized plans of care through interdisciplinary team meetings
• Maintain accurate, timely, and compliant clinical documentation
• Coordinate and oversee physical patient care, providing instruction to caregivers, volunteers, LPNs, and home health aides
• Supervise and document home health aide plans of care every two weeks
• Participate in rotating 24‑hour on‑call coverage
• Provide support to patients and families during the time of death and throughout the bereavement period
• Participate in staff orientation, in‑service training, and quality improvement initiatives
• Assist with community education, outreach, and hospice service awareness as needed
• Represent the hospice in professional and community activities
• Maintain compliance with agency policies and state and federal regulations
Physical Requirements
• Frequently lift up to 40 lbs; occasionally up to 50 lbs unassisted
• Ability to carry medical supplies up to 30 lbs
• Push/pull equipment related to patient care
• Bend, reach, twist, and perform repetitive motions required for patient care
• Walk, stand, climb stairs, curbs, and access patient homes

Required Skills & Experience
• Active Registered Nurse license per Pennsylvania state requirements
• Graduate of an accredited school of nursing (National League of Nursing or equivalent)
• Valid Pennsylvania driver’s license, current auto insurance, and reliable transportation
• Ability to work independently while collaborating within an interdisciplinary care team
• Strong assessment, documentation, and care coordination skills
• Strong clinical judgment and nursing practice skills
• Excellent communication and interdisciplinary teamwork abilities
• Ability to manage time, caseloads, and priorities effectively
• Compassion and emotional resilience when supporting patients and families
• Ability to educate caregivers, staff, and community members
• Commitment to confidentiality and ethical patient care
• Basic computer proficiency
Nice to Have Skills & Experience
• Hospice or home health experience with terminally ill patients
• Bachelor of Science in Nursing (BSN)
• Certified Hospice and Palliative Care Nurse (CHPN)
