Job Description
We are seeking an experienced Healthcare Project Manager with Epic expertise to support multiple upcoming Community Connect implementations. This individual will lead the rollout of new affiliate sites, partnering closely with operational leadership, clinical stakeholders, and third-party vendors to ensure successful implementation, adoption, and delivery.
Lead end-to-end project management for 3–4 Community Connect implementations, focused on onboarding new sites
Partner with operational leaders (implementation champions), IT teams, and vendors to drive alignment and execution
Develop and manage detailed project plans, timelines, and deliverables within Workfront
Facilitate key project cadences including:
Planning sessions
Discovery meetings
Technical walkthroughs
Track and report on project status, proactively identifying risks, dependencies, and escalations
Maintain strong documentation across all phases (plans, decisions, risks, workflows)
Coordinate across technical infrastructure, application, and operational workstreams
Ensure readiness across all phases including planning, build, testing, and go-live (aligned to Epic implementation lifecycle practices)

Required Skills & Experience
5+ years of Project Management experience in healthcare IT
Proven experience supporting a Community Connect implementations
Strong understanding of Epic workflows and the implementation lifecycle
Experience working with:
- Cross-functional healthcare teams (clinical, operational, IT)
- Third-party vendors and external partners
- Demonstrated ability to manage multiple concurrent projects
- Strong organization, documentation, and communication skills
- Experience with project tracking tools (Workfront preferred)
- Knowledge of technical infrastructure considerations within healthcare implementations
Nice to Have Skills & Experience
Experience with FQHC (Federally Qualified Health Center) environments
Prior experience overseeing new site / affiliate implementations
Familiarity with Epic Community Connect models and governance
