Job Description

The Intake Clinician will serve as the first point of contact for patients entering both the general outpatient mental health clinic and the dementia-focused clinic, conducting comprehensive biopsychosocial assessments to determine appropriate levels of care and treatment pathways. On a daily basis, this role completes diagnostic screenings, gathers clinical history, assesses risk, and identifies patient needs across a wide range of psychiatric presentations, including cognitive impairment and dementia-related concerns. The clinician collaborates closely with psychiatrists, APNs, social workers, peer support specialists, and interdisciplinary team members to coordinate referrals, connect patients to services, and ensure smooth transitions into ongoing treatment. This role requires strong clinical judgment, adaptability, and the ability to work with diverse, acute, and aging populations, while maintaining accurate documentation and providing compassionate, patient-centered care in a fast-paced outpatient setting.

Required Skills & Experience

· 2+ years’ experience postgrad in any mental health related field
· Master’s degree in related mental health field
Licensed in NJ (LCSW or LPC)
