Job Description

Day-to-Day:

This is a fully on‑site, patient‑facing role where RN case managers are assigned to specific units—including the Weaver Tower (Neuro/Stroke and Oncology in partnership with MD Anderson), the Heart Hospital (Cardiovascular and Open Heart), or the Main Tower (Med‑Surg, Orthopedics, and Bariatrics)—and work collaboratively within an interdisciplinary team of physicians, bedside RNs, social workers, and care coordination assistants, participating in or leading daily progression‑of‑care rounds, with no people management responsibilities. Duties include high risk screening, assessment, discharge planning, and coordination of post-acute care services.

Required Skills & Experience

Must-Haves:

RN with Case Management experience (2+ years ideal)
Strong payer / insurance knowledge
Transitions of care experience
Comfortable in fast‑paced, high‑throughput environment
Confident communicator with physicians
Experience leading or contributing to interdisciplinary rounds
