Job Description
Day-to-Day Responsibilities:
Build and maintain patient medical charts within the EHR system
Accurately enter and update patient demographic information
Assign insurance information, ensuring correct entry into the system
Upload, document, and organize provider notes and clinical documentation within patient charts
Assist with insurance filing by ensuring documentation is complete and properly attached
Review and interpret handwritten or dictated provider notes for accurate data entry
Ensure all records meet compliance, accuracy, and confidentiality standards
Communicate with clinical and administrative staff to clarify missing or unclear information
Maintain organized and timely processing of all incoming documentation

Required Skills & Experience
Required Qualifications
High school diploma or equivalent
2+ year of data entry experience in a healthcare or medical office setting
Strong attention to detail with high accuracy rates
Proficiency in Microsoft Office (Excel, Word) and data entry systems
Very reliable with strong communication skills
