Ensure Quality, Safety, and Regulatory ComplianceMaintain accurate, timely documentation in alignment with regulations and internal policies.Follow professional standards of practice, including providing direct patient care when required.Monitor and coordinate care across all disciplines within assigned caseloads.Participate in quality improvement efforts, evaluating clinical data to identify trends and areas for improvement.4. Direct Patient Care & Timely DocumentationConduct initial and ongoing clinical assessments to evaluate patient needs and progress.Create and update individualized care plans in collaboration with the interdisciplinary team.Investigate and resolve patient or family concerns as assigned by leadership.Participate in the on-call rotation.Ensure all case-management tasks are completed for patients in the assigned geographic area.5. Support Admissions & Cross-Department CollaborationCoordinate with patient representatives, advocacy groups, and other stakeholders involved in the case.Support new admissions and ensure smooth onboarding into agency services.Participate in weekly meetings with clinical leadership.Provide patient and family education related to conditions, medications, treatments, and preventive care.Assist in crisis situations by coordinating resources, providing intervention strategies, and notifying necessary departments.Compensation: $90,000 to $100,000. Exact compensation may vary based on several factors, including skills, experience, and education.
