Job Description

Insight Global is seeking a Care Coordinator for one of their Social Service clients in South Jersey.
The Care Coordinator is a Master’s-level clinician (LSW preferred) responsible for providing comprehensive care coordination, clinical oversight, crisis triage support, and ongoing case management for individuals served by the Community Crisis Response Team (CCRT). Working under the supervision of the Lead Clinician (LCSW), this individual conducts brief assessments following crisis interventions, supports the development of individualized care plans, and ensures services are delivered in accordance with trauma-informed, recovery-oriented practices. The Care Coordinator provides clinical guidance, supervision, and support to Peer Specialists while coordinating referrals and warm handoffs to behavioral health, medical, and community-based social service providers. This role is responsible for conducting timely follow-up within 24–72 hours of a crisis response, monitoring client engagement, addressing barriers to care, and facilitating re-engagement efforts as needed to promote long-term stability and recovery. In addition, the Care Coordinator participates in case reviews, multidisciplinary team meetings, and care coordination efforts, serving as a liaison between clients, families, community partners, and healthcare providers. The position also supports program operations through accurate documentation, service tracking, quality improvement initiatives, data reporting, training participation, crisis response planning, and team debriefings. This role requires frequent travel throughout the community, the ability to sit or stand for extended periods, and the ability to lift up to 50 pounds while supporting field-based crisis response and recovery services.

Required Skills & Experience

• Minimum 3 years of experience in behavioral health, substance use, or co-occurring disorders
• Experience in crisis response, emergency services, or community-based care
• Knowledge of crisis intervention, trauma-informed care, and recovery-oriented practices
• Ability to provide clinical oversight and support multidisciplinary teams
• Strong communication, documentation, and decision-making skills
• Master’s Degree in Social Work or related field required
Active New Jersey Licensed Social Worker (LSW)

• Prior supervisory or team leadership experience
Bachelor's Degree in related field
